Step by step
You will receive an email to confirm that your GP has submitted a referral. This will include a referral number, which helps us find your details more easily.
Preparing for your child's assessment
Complete the pre-assessment questionnaires
Before your child’s assessment, there will be several questionnaires to complete. We’ll need these at least 48 hours before your appointment.
For children aged five to ten there are:
1 questionnaire for your child - in your portal for your child to complete
4 questionnaires for you about your child - in your portal for you to complete
2 questionnaires for your child's teacher - link sent by email, for you to send to your informant
For children aged 11 and above:
2 questionnaires for your child - in your portal for your child to complete
4 questionnaires for you about your child - in your portal for you to complete
2 questionnaires for your child's teacher - link sent by email, for you to send to the school
Additional supporting information
Certain documents such as school reports, speech and language, psychology or other medical reports can help your child’s clinician better understand their developmental history and current behaviour.
Your child's assessment
Two specialist clinicians will work with you and your child as part of the assessment. Each clinician focuses on a different part of the picture, so we can understand your child’s strengths, needs and day to day experiences as fully as possible.
