NHS Patient Services Team call patient to:

• Explain assessment processes/answer questions/check patient info correct

• Provide access to CP NHS Patient portal – via email

• Complete remote assessment suitability questionnaire (if commissioned)

• Book assessment date – usually within two–three

  weeks of the call

• Book feedback session

Any missing information gathered by CP’s NHS team.

Login details for Patient portal emailed to parent/carer

Pre-assessment questionnaires emailed and/or made available on Patient portal:

• Caregiver: Conners-4PS, SDQ, AQ-10 child/adolescent, R-CADS, ‘Your Child’s Background’

• Self-Report: Conners Index Self Report, R-CADS (8+), SDQ (11+)

• School: SDQ Teacher, School Questionnaire, Conners 3-S

Specialist Clinician conducts Young-DIVA assessment (Approx. two hours).

Specialist Clinicians are selected from our diverse Multidisciplinary Team (MDT), with the appropriate practitioner allocated according to service and clinical need.

CP team contacts school/family/other professionals involved in the referral for further info before confirming diagnosis outcome if required.

Feedback call with Parent/carer and young person when appropriate. (Approx. 30 mins within two weeks of assessment)

Assessment report including person centred care and treatment recommendations is sent to parent/carer and referrer within four weeks of feedback session.

Onward referrals/further assessment, treatment and support is provided in line with contract and local pathways.

Initial medication appointment with Specialist Clinician within four weeks of feedback session: physical health check, detailed discussion of meds options and compliance (Approx. one hour)

Specialist Clinicians are selected from our diverse Multidisciplinary Team (MDT), with the appropriate practitioner allocated according to service and clinical need.

Follow-up titration with a Specialist Clinician within four weeks of initiation, including physical health and compliance checks (approx. 30 minutes), followed by ongoing titration appointments until the patient is stabilised.

Specialist Clinicians are selected from our diverse Multidisciplinary Team (MDT), with the appropriate practitioner allocated according to service and clinical need.

Option 1

If shared care is in place locally, we establish an agreement with the GP. The GP prescribes and undertakes physical health checks. We continue as the specialist service (bi-annual reviews, GP advice/guidance) or the referring Trust does so by agreement.

Option 2

If shared care is not in place locally, we provide the full pathway: physical health monitoring, prescribing, and reviews.

Option 3

We stabilise the patient, then return care directly to the referring trust.
